Step 1: Enter Basic Information
- Age: Enter your age in years. Ensure that the age entered is between 40 and 79, as the calculator is designed for this age range.
- Sex: Select your sex from the available options. You can choose either “Male” or “Female.”
Step 2: Select Race
- Race: Choose the appropriate race category for yourself from the options provided: “White,” “African American,” or “Other.”
Step 3: Enter Cholesterol Levels
- Total Cholesterol: Enter your total cholesterol level in mg/dL. The allowed range is between 130 and 320.
- HDL Cholesterol: Enter your HDL cholesterol level in mg/dL. Ensure the value is between 20 and 100.
Step 4: Enter Blood Pressure Information
- Systolic Blood Pressure: Enter your systolic blood pressure in mmHg. The value should be between 90 and 200.
- On Blood Pressure Medication?: Indicate whether you are currently taking blood pressure medications by selecting “Yes” or “No.”
Step 5: Enter Additional Health Information
- Diabetes: Indicate if you have diabetes by selecting “Yes” or “No.”

- If your risk is 20% or higher, your risk level will be labeled as “High Risk.”
- If your risk is between 7.5% and 19.9%, your risk level will be labeled as “Intermediate Risk.”
- If your risk is less than 7.5%, your risk level will be labeled as “Low Risk.”
