This Opioid Conversion Calculator is designed to help you convert between different opioid medications based on their morphine equivalent dose. Use the following guide to input your data correctly and interpret the calculated results.
Step 1: Select the Current Opioid
- Locate the Current Opioid dropdown menu.
- Click to view the options and choose the opioid you are currently using. The options include:
- Morphine
- Oxycodone
- Hydromorphone
- Fentanyl (mcg/hr)
- Hydrocodone
- Codeine
- Tramadol
Step 2: Enter the Current Daily Dose
- Find the Current Daily Dose field.
- Enter your current daily dose in milligrams (mg) into the field. Note that the dose must be a non-negative number and can include decimal points (e.g., 10.5).
Step 3: Select the Target Opioid
- Navigate to the Target Opioid dropdown menu.
- Choose the opioid you wish to convert to. Options are the same as the Current Opioid list:
- Morphine
- Oxycodone
- Hydromorphone
- Fentanyl (mcg/hr)
- Hydrocodone
- Codeine
- Tramadol
Step 4: Review the Calculated Results
- Once you have entered the necessary information, the calculator will automatically provide two important results.
- Morphine Equivalent Daily Dose (MEDD): This value represents the equivalent dose of morphine (in mg) based on the opioid and dose you entered. It is calculated according to established conversion factors.
- Converted Target Daily Dose: This result provides the daily dose (in mg) of the target opioid that is equivalent to the MEDD. It helps in safely converting from your current opioid therapy to the target opioid.
